
Community Reach is pleased to operate the Accessible Transit (AT) program on behalf of the Town of Midland. This service is available to any resident of Midland who cannot use the Town’s public transit due to physical, cognitive, visual or sensory disabilities. We use town-owned accessible vehicles to provide rides for medical appointments, shopping and social activities within the Towns of Midland and Penetanguishene.
Applications must be accompanied by a medical form completed by a medical professional such as a licensed physician, licensed occupational therapist, nurse/nurse practitioner, optometrist or registered physical therapist.
- For Midland residents ONLY unable to use public transit due to a physical, cognitive, visual, or sensory disability

- Service operates Monday – Friday, 6:30 am to 5:30 pm; Saturday 8:30 am – 4:30 pm
- Office hours for ride booking: Monday – Friday, 8:30 am – 4 pm; Saturday 8:30 am – 1:30 pm

Our ONLINE REGISTRATION FORM can be filled out online and will be automatically be emailed to our office once completed. Our MEDICAL ASSESSMENT form needs to be downloaded and filled out and signed by a medical professional. It can be sent via email to admin@communityreach.ca, faxed or dropped off at our office. A PDF copy of our REGISTRATION FORM can also be downloaded and returned to our office.
